What game was that from?

Brisbane City v Palm Beach Sharks, published May 12 2013, so I assume it's the Round 8 fixture of NPL Queensland.

Close but i think this one is worse because a goal was scored.

I'm not sure he's ruling for an offside. I hope not anyway.

When an assistant raises the flag and holds it in the air upwards at a 45 degree angle, it means an offside on the far side of the pitch. If it was an offside right next to him, he would flag it by pointing the flag 45 degrees towards the ground.

This looks like he is pointing vertically, which is just a foul.

Either way, the video doesn't stay on him for long enough to be able to properly judge what he is flagging.

His eyes are on the ball-player then his focus shifts to the ball in the air and finally to the last defender. I think a refresher course is in order.

Trying to give him the benefit of the doubt, but yeah, it was a terrible decision.
