I wouldn't call the Rafael one a dive, just saw the challenge coming and got out of the way, it was a rash challenge. However, I have not seen a replay so will stand corrected.

The pull from Kagawa, obvious foul. The ref was right behind him though from the replay, I think that was just bad timing, there was no way he could see it, he was at the worst possible angle for the ref.
