What do you do when you get the feeling you've offended someone, but don't know why?

Unfortunately I need to know why. It must have come from curiosity at a young age why someone is feeling so. I therefore try to get to the bottom of it and wonder why they're annoyed or upset. I've learnt that the vast majority of times it's them and not me. I subtly bring up certain things from the past and notice they've either forgotten it or they are still so overly offended that it's laughable. Learning more and more that it's their issue and not mine. I was simply stating an opinion that was fair and balanced or doing my job professionally and the ESL lesson is well meant and tailored. Questions are asked on how to enhance and they're lazy to even try to contribute. What more can you do?

I guess it depends on how serious you think the offence may be and how close you are to that person.

Never post anything you wouldn't actually say in your day to day life.  I've met a fair few people who post vile shit online yet they wouldn't dare say it in person . 
Don't trust so easily is another one. Got burnt a few times with that one
And lastly never assume about anyone
